Why We Recommend a Consultation First — Rather Than Direct Booking

Some clinics let you book a penile filler appointment — or even surgery — directly online, the way you might book a haircut. At London Andrology, we deliberately do not. Every treatment pathway at our clinic begins with a specialist consultation, in person or by video.

Patients occasionally ask why we add this step when other providers don’t. The answer is simple: it is safer, it produces better results, and it sometimes uncovers medical issues that matter far more than the procedure you came in for.

1. Size Concerns Can Mask an Underlying Condition

This is the most important reason, and the one direct-booking providers cannot address.

A significant number of men who approach us about enhancement are, on assessment, also experiencing an unrecognised or unmentioned medical issue — most commonly erectile dysfunction, but also Peyronie’s disease, hormonal issues such as low testosterone, or genuine anatomical abnormality.

A man might notice his penis “looks smaller” and seek enlargement, when the underlying change is reduced erectile quality — which can itself be an early signal of cardiovascular disease, diabetes, or hormonal imbalance. Treating the cosmetic concern while missing the medical one is not just poor practice; it can delay diagnosis of something serious.

A consultation with an andrologist screens for exactly this. A booking form does not.

2. The Right Procedure Depends on Your Anatomy — Not the Menu

Fillers, fat transfer, ligamentolysis, traction therapy — these are not interchangeable products at different price points. Which is appropriate (if any) depends on your anatomy, your goals, your health history, and trade-offs such as reversibility versus longevity.

Direct booking assumes you have correctly diagnosed yourself and correctly selected your own treatment. In our experience, after a proper consultation a meaningful proportion of men choose a different option from the one they arrived intending to book — and some choose no procedure at all. Both are good outcomes.

3. Realistic Expectations Are Set Before Money Changes Hands

The strongest predictor of satisfaction after enhancement procedures is not centimetres gained — it is whether expectations were accurate beforehand¹. A consultation is where we explain, honestly:

  • What a procedure can achieve for your anatomy
  • What it cannot achieve (for example, why lengthening surgery primarily increases flaccid rather than erect length)
  • Risks, recovery, maintenance, and true long-term costs

If a clinic’s first interaction with you is taking payment, that conversation has not happened.

4. Screening Protects Vulnerable Patients

For a small but important group of men, distress about penile size reflects body dysmorphic disorder (BDD) or another psychological concern — and for these patients, a procedure typically does not resolve the distress and can worsen it. Professional guidance is clear that screening and, where appropriate, psychological support should come first.

This assessment requires a clinician, time, and care. It is a core part of our consultation, and one of the clearest reasons responsible clinics do not sell these procedures off the shelf.

5. It Is Also a Regulatory and Ethical Standard

Informed consent is not a signature on the day of a procedure — it is a process. UK professional standards expect patients to receive balanced information, time to reflect, and the opportunity to ask questions before committing to a cosmetic intervention.
